Referrals may be made by family members, physicians, nursing facilities, assisted living facilities, social workers, case managers and any other involved party who feels an individual may benefit from our services. We offer a free initial assessment by a mental health professional. Horizons accepts Medicare and most private insurance.
Horizons Geri-Psych Center offers:
Patients participate with a group of peers who are similar in age and life experiences. Families are included in the therapeutic process. Aftercare planning is also provided to maintain improvement made in treatment.
Horizons Geri-Psych Center puts an emphasis on family involvement. Family members are encouraged to attend weekly family conferences. These conferences give the family, patient and Horizons staff the opportunity to sit down and discuss the individual’s progress in treatment and the family is able to ask any questions they may have. Daily visiting hours are available.
